Bridge Review Checklist
Required for all bridge crossings and pipe crossings with diameter 36-inches or greater. Form lists the minimum plan requirements for bridge crossings.

Citizens Use Permit
Needed for temporary or permanent changes in roadway operation, including street closures for block parties and other events.

Subdivision Improvement Agreement
Required for all subdivisions. The Improvement Agreement outlines the Developer’s and ACHD’s responsibilities for the development.

Subdivision Inspection Agreement
Required for subdivisions having new public streets. The Inspection Agreement allows ACHD to enter the property to complete roadway inspections, sets the completion date and identifies the amount of the inspection deposit and how inspection fees are charged and refunded.
